Student Agreement Form

Arizona Institute for Health Sciences (AIHS)

This Agreement is made between the Arizona Institute for Health Sciences ("AIHS") and the undersigned student ("Student").

1. Materials Usage

The Student agrees not to download, copy, or record any materials provided during the course of study at AIHS. All materials are the exclusive property of AIHS and are provided solely for personal use in the context of the training.

2. Fee Agreement

The Student acknowledges that all fees paid to AIHS are non-refundable as stipulated in the Student Handbook, which the Student has either received or has had the opportunity to review.

3. Learning and Performance

The Student is responsible for completing all tasks assigned within the AIHS learning management system and agrees to proactively contact the school for any concerns related to the materials or other processes involved in the training.

The Student understands that scheduling and participating in practice tests are crucial in preparing for the official examination.

The Student commits to attending all scheduled review sessions as part of their preparation and training.

4. Handbook Compliance

The Student agrees to read, understand, and comply with the contents of the AIHS Student Handbook. Compliance with the handbook is essential for the successful completion of the training program.

5. Acknowledgment

The Student acknowledges having read and understood this Agreement prior to signing and agrees to be bound by its terms as a condition of their enrollment and continued participation at AIHS.
